A Patient Centered Medical Home (PCMH) is an approach to medical care in which both the doctor AND the patient take an active roll in the health care equation.

It isn't enough for the doctor to simply say take this, or do that, and then have a positive outcome realized.

In the PCMH relationship, the patient comes to the doctor with a health issue, and the doctor works with the patient to identify the source of the problem. But it doesn't end there. PCMH requires that the patient follow the prescribed treatment and also provide feedback to the physician relating to the success of the care. It also initiates the need for follow up care to bolster even greater success in the treatment.

The PCMH model also positions the primary care physician as the first point of contact in ALL of your medical care. In other words, before going to the emergency room or urgent care, a simple call to your PCMH doctor may save you time and expense, by having your doctor assess your problem by phone and advise you of the best course of care.

Of course in an emergency where life is in jeopardy proceed to the nearest emergency room.

As the patient portion of the PCMH model, it is your responsibility to make timely appointments for your care, keep track of your medications so that you don't run out at times that are outside of office hours, and provide the doctor with an accurate assessment of your current health conditions each time your visit.
